Laser procedures demonstrate long-lasting, sometimes even permanent results in the treatment of acne scars. If injectable cosmetic fillers are used in acne scars, the results generally last several months before re-treatment is necessary.

Deep acne scarring may require surgical techniques that remove scar tissue, dermabrasion, or temporary or permanent dermal fillers or fat injections to smooth pits and depressions.

Surgical excisions can be performed on small, well-defined, deep acne scars and can give fairly satisfactory results. It is carried out using a small punch biopsy tool which has a circular blade of varying widths. Local anaesthesia is applied to the area.

Often, the best treatment of severe lesions is only a  combination of subcision and small-volume fat transfer, acne laser treatment, dermabrasion, and excision or grafting.
